NJ Department of Education released the Student -Athlete Cardiac Assessment professional development module for physician and physician assistants. All physicals (PPE) may only be completed by a licensed physician, APN or PA that has completed the module. It is recommended that you verify that your medical provider has completed the module before scheduling an appointment.

This is a professional development course to be completed by the health care provider performing the physical exam. If page four of the physical is not signed by physician that he/she completed the cardiac assessment course the district cannot accept the physical.
Physical form must be completed correctly in order to participate in athletics. Please refer to the physical checklist instructions to ensure your physical is not returned and student is cleared for participation in a timely manner.
Check with your insurance carrier regarding when in the calendar year a new exam is allowed. Some carriers allow a new physical to be completed when the calendar year changes some require 365 days.
