Dear Parents/Guardians of Present 5th Grade Students:                             



     Certain vaccinations are required for a child to attend schools in New Jersey.   As of  January 2008, a new NJ state law (N.J.A.C. 8:57 - 4.10 and 4.20) requires all students born after January 1, 1997 and who will be in Sixth Grade (or a comparable age level special education program with an unassigned grade) on or after September 1, 2008 to receive one dose of tetanus, diphtheria, pertussis vaccine (otherwise known as Tdap) and one dose of a meningococcal (meningitis) containing vaccine.



     As this law will now apply to your present 5th grader, please see to it that your child receive these immunizations by September.  Students who are not compliant with this new law can be excluded from Middletown Township Public Schools.  Please send the updated immunization record to the school health office so that I may update your child’s health record.
