• When does my physical have to be returned to my coach?

    During the summer students must submit their physical as soon as they have a new exam. No athlete is to participate in summer workouts without a valid physical.
     
    Physicals should be handed in once completed, to ensure the school physician can approve it prior to the start of your season.
     
    Athletic participation registration is now required on the Family ID website. Physicals will be required to be in paper format.
     
    When a physical is after the registration deadline students must complete Family ID registration by deadlines posted.

  • How do I know if I need a physical?

    You can check the last physical on file on your Family Id account. It is a good idea to keep a copy of your physical and your physician is required to.
    Please do not hand in a valid physical that is already on file, thank you.

  • Does my physician have to complete both pages three and four of the PPE?

    Yes as per the department of education, the physician or qualified health care provider must complete and sign both pages of the physical portion of the PPE.

    The physician must sign page four twice. Once with the date of the exam and once that the physician completed the cardiac module with the date it was completed, not the date of the physical exam. Page four must be stamped by the office.

  • Can I drop forms off to the main office?

    NO. All physicals must be returned to your coaching staff. Check schedules to meet with coaches. 
    No emails, scans or faxes of physicals either.

  • Where are the ImPACT instructions?

    ImPACT instructions were distributed at your sports' interest meeting. They are not available online. If you are unsure if you need to test check the lists posted or with your coach. Coaches were given ImPACT instructions they are not available online.

  • Do I have to do the ImPACT test?

    Yes, baseline ImPACT testing is a requirement of athletic participation.
     
    All incoming freshmen participating in athletics and or cheerleading must complete the ImPACT test.
     
    Generally students take the baseline during their freshmen year and re test during their junior year. This not always the case. If it has been two years since your baselnie you need to test.
     
    If you are a "new" student, transfer or have not played a sport at HS North you need to test.
